The above statement is a good point!! I did think that the CCG's are being run by local GP's in a coalition style approach, although I am aware that locally the GP's who are taking over have recruited from staff within the exiting PCT's because they don't necessarily have the expertise as yet to manage the commissioning etc of local services. Although it could be argued that PCT commissioners are still very much novices in the new business practices associated with tendering and procuring. Another point to make in this mix is that GP's will be commissioners and providers and there has to be a tension and potential for less than transparent practices in the mix. A really scary time for the NHS over the next few years and perhaps it will have to change name from National Health Service to Local Health and Wellbeing Ltd [LHWL]???? And a really bad time for people who have worked and done their best to continue to deliver high quality care through a period of massive change and uncertainty. The old style NHS ran on the goodwill of most staff giving over and above the call of duty. From the posts I have read in a lot of the discussion threads this continues to be the case. But how long can it continue in a profit/loss driven environment where staff and their skills are not valued?

It is clear that this move is about so much more than the NHS pension scheme. I just wanted to say in the bigger picture I think the Government want to make it easier for tenders to be awarded to private companies and for staff to be TUPE'd out of NHS Providers. When the CCG's take over the management of the NHS at a local level in 2013 we will start to see what priorities GP's have for services in their local areas. I think to some extent staff in the acute sector have been protected from the effects of the service they provide being "out to tender" and their employing provider not winning the tender. However, that may be about to change radically

PS I know KSF's separate from AfC but they were launched at about the same time and so further complicated the AfC implementation. It was usual where I work for people to confuse the two initiatives and get very anxious about what it meant for them

Comment on: Plans to expand NHS overseas branches
“The NHS has a world-class reputation, and this exciting development will make the most of that to deliver real benefits for both patients and taxpayers.” Which patients and taxpayers are these? I love the optimistic point that David Stout doesn't THINK the initiative will divert attention away from running services in this country. My perspective is that money speaks far louder than health needs ever could and if there are big bucks to be made abroad then that is where all the effort will be diverted to.

I also agree in relation to getting basic pay when off sick, if you are not on duty for unsocial hours then why would you need an enhancements? When I think about the time, energy and anxiety that went into implementing AfC. Rewriting job descriptions, creating person specs, setting up and running panels to agree banding, dealing with appeals and disputes, writing KSF's etc etc, how expensive will it have been if they just discard it all only a few years later. Surely there must be a better way forward than throwing the baby out with the bathwater
