Phillip Collings - for Services to Increasing Care Closer to Home

Philip has focused on community rehabilitation of individuals with serious and enduring mental health problems. He set up the first assertive outreach team in Wales 2001, and continues to lead this extraordinary multi-disciplinary team, which provides truly client-centred care to people with severe and enduring mental health problems. Remarkably over the past two years he and his team have virtually eliminated hospital admissions in this client group. 

Philip is a modest and inspirational pioneer. He shares his work with colleagues across Wales and has supported the development of further teams throughout Hywel Dda NHS Trust. He is a founding member of South West Wales Assertive Outreach Forum.

Philip’s unit was awarded Practice Development Unit status in 2007 by Leeds University.  This was the first and only accredited PDU assertive outreach team in the UK and achieved reaccreditation in 2009.
