Maintain pressure on reforms to protect NHSKidney patients to get personal care plans
All adult patients with chronic kidney disease in England are to receive a new personalised system of care, the national director for kidney care Dr Donal O’Donoghue has announced.
CKD patients are to be given a personal folder – My Kidney Care Plan – which is designed to organise support around individual needs. The folder, which has been piloted by over 100 patients, will be rolled out over the next three months.
It contains ‘prompts for discussion’ with a specialist nurse on subjects ranging from treatment choices to social activities, and includes a section for keeping notes.
Jane McDonald, lead nurse in renal services at Salford Royal NHS Foundation Trust, which was one of the pilot sites, said: ‘For some time now nursing documentation has centred on capturing information about the dialysis therapy delivered.
‘The Kidney Care Plan focuses instead on outcomes, goals or information needs that the patients themselves identify as their priority,’ she said. ‘Together, nurse and patient are able to have meaningful and productive discussions, identify focused support or care to be delivered, all in a plan held by the patients themselves.’
