Advice for care of multiple conditions

A report by the National Council for Palliative Care aims to improve end-of-life care for older people with multiple conditions

Multiple Conditions, Multiple Challenges, says there is confusion about caring for those with complex health problems, and warns: ‘It might be perfectly possible to provide excellent management of a specific condition while making the individual’s quality of life worse.’

The report recommends more advance planning based on regular multidisciplinary assessments and stresses the importance of good communication with relatives and agencies. It also calls for more community geriatricians to assess frail older people in care homes.
