MHRA issues IV set warning

The Medicines and Healthcare Products Regulatory Agency has issued a warning over certain lots of level one normothermic IV fluid administration sets for use with fast flow fluid warming units, manufactured by Smiths Medical.

The affected product codes are D-60HL and DI-60HL.

The MHRA said the heat exchanger assembly on the sets may be too long, and therefore may not fit into device’s fast flow fluid warmers.

There has been a report of a death associated with the inability to find and use a set with the correct length heat exchanger during an emergency blood transfusion.
