Guidance on anxiety management published

NICE has published for consultation draft guidance on managing anxiety. The draft update covers the management of adults with a generalised anxiety disorder. 

It recommends that antipsychotics should not be offered for the treatment of GAD in primary care and says those whose symptoms do not improve with education and active monitoring, should be offered individual pure self-help, individual guided self-help or psycho-educational groups as first line treatments.

  • Suspect that "individual pure self help" means referral for individual psychological work,which is fine ,but this will then fall to secondary care cmhts, further deflecting us fromworking with people with enduring illness.

  • The little bit of information above does not really tell us that much; however, it sounds very 'wooly' to meand, yes, it does sound like a bit of a 'cop out'.

  • I have some ideas for education 'self-help' when it comes to anxiety:
    1. Beware the punishment mentality. Do not try to punish others by saying, "You had me so worried!" Don't be judge, jury and executioner over someone else.
    2. Do not punish yourself overly much for things out of your control. Make sure the punishment fits the crime. If you have done something wrong, admit it, fix it, and then the anxiety over it will resolve.
    3. Remember that anxiety is sapping away the strength you need to get up and do something. Take projects that cause anxiety in small bites. Look for short-term successes and rewards that will fuel you to continue to the big picture.
    4. If you are not outraged, you are not paying attention! However, you have to prioritize what is of greatest concern, and what, if anything you can do about it. You have to let go of things you can't change. That's why people love the Serenity Prayer and the Desiderata.
    5. Prevention is always better than the cure. Get up every day and think what you can do to turn your load from putting out fires to keeping matches away from children. Think of every problem in terms of the Fence or the Ambulance.

  • As i sufferer from anxiety I find the article quite interesting. I was fortunate in that my primary HCT has a balanced view on this problem; i was offered both psychotropic intervention and self help therapies. The meds brought down the anxiety levels enabling me to pursue the self-help. not long after i was able to come off the meds and now use non-medical self-help therapies when needed
