Tailored primary care service for care homes launched

A new primary care service that will offer care 12 hours a day, 365 days a year, is being launched for nursing homes residents across the West London borough of Ealing.

Ealing Clinical Commissioning Group said it had introduced the service in response to the increasing complexity and needs of local nursing home residents.

The Argyle Road Surgery will provide the service to over 1,000 residents across 22 nursing homes.

It will recruit staff to deliver the service who will provide frequent reviews and follow-up of patients, rapid response to requests for urgent visits, and a specific focus on improving end of life care.

Dedicated medicines management support to those patients who are on multiple medications will also be provided, said the CCG.

Dr Shanker Vijay, the project’s clinical lead, said the service had been developed with input from patient representatives, carers, nursing home staff and local clinicians.

“The aim was to design a service that would meet the needs of all those involved,” he said. “Our sincere hope is that providing this enhanced primary care service for nursing home residents will improve the quality of their care and reduce the number of avoidable emergency admissions.”

Ealing CCG chair Dr Mohini Parmar added: “This is an innovative service and, to the best of our knowledge, we are one of the first CCGs to design a dedicated service specifically for nursing home residents.”

  • How can this be better than the existing 24/7 service which ALL Nursing Homes have to provide already I wonder ?
    I would be most interested what the CCG have agreed to pay for this "part time" service so that we might compare it with the full time Care Packages already forced on Homes by the CCGs
