Nursing award for patient passport scheme that gives info about patients with learning disabilities

A scheme that gives hospital staff vital information about patients with learning disabilities has won a £2,000 Foundation of Nursing Award.

Wandsworth Primary Care Teaching NHS Trust’s passport scheme provides vital information in case patients are unable to communicate in any other way.

Passports are ‘owned’ by the patient, who controls the information they hold.

The scheme is a result of work by the PCT’s Wandsworth Community Learning Disability Team and St George’s Healthcare NHS Trust.
