Designing an electronic holistic needs assessment form


I am a nurse working within an IT department. Part of my role is helping clinical services to design electronic versions of paper based forms. I agree that it frequently takes longer than expected and user feedback is essential to the process. I have been working with our community teams to create forms that can be completed on a tablet device in the patient's home and which are sent directly from the device to the Electronic Patient Record (EPR). Like Leeds, our local GPs can access this information in the EPR. We can also email documents from the EPR using a secure email (NHS mail) to GP's tat are outside our area. This information sharing has had a positive impact on the quality of communication between services and benefited of patient care.
